What is PCMH?

From NCQA: PCMH stands for Patient Centered Medical Home.

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into “what patients want it to be.” Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.

NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely-used way to transform primary care practices into medical homes.

Patient-Centered Satisfaction, Clinical Quality, Care Coordination, Utilization Measures, and Evidence-Based Care.

As part of our commitment to becoming a Patient-Centered Medical Home, we selected measures to include Clinical Quality, Care Coordination, and Utilization Measures. We worked hard as a practice to improve care for our patients in these areas.
Also, as a means of quality improvement for our patients, we emailed our patients seen in a recent 6-month period requesting feedback on their office visit by completing an online survey. We were very pleased with the level of satisfaction that our patients expressed about the care they received! For the results of this survey and measures, click the links below.
